New patients will be asked to complete a health questionnaire, as it may be some time before notes arrive from a previous doctor.
The practice area covers Virginia Water, including the Wentworth Estate and Knowle Hill.
The area is bounded by:
To the West by the A30.
To the East by the line of the M25 at Thorpe Green and the M3 at Bridge Lane.
To the North is a line from the end of Callow Hill, where it becomes Bakeham Lane and to Luddington Avenue and Hurst Lane off Stroude Road.
The southern edge is formed by the line of West Drive and Beechwood Road on the Wentworth Estate.
If you change name, address or telephone number, please let our receptionists know. If you move outside the practice area you will need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.

You may print off a Registration Form and a New Patient Questionnaire, fill them in and bring them in with you on your first visit to the practice, together with proof of identity and address.

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
